Job summary

PALM Health, LLC in St. Louis, MO is seeking a Patient Liaison/Care Coordinator to be the primary contact for a designated medical provider and to coordinate day-to-day tasks.

You’ll ensure appointments are organized with current patient information and deliver exceptional patient education and follow-ups. You will support labs, supplements, and referrals, maintain warm, clear communication, and help relay wellness recommendations as part of our whole-person approach.

Qualifications

• Bachelor’s degree (B.S. or B.A.) or LPN license required.
• One year medical office experience required; will consider other combinations.
• Experience with medical terminology and working in a clinic or hospital setting required.
• Strong computer and word processing skills required.
• EMR and scheduling experience preferred.

Responsibilities

• Serve as the primary point of contact for a designated medical provider and coordinate day-to-day tasks.
• Ensure providers are prepared for appointments with organized schedules and up-to-date patient info.
• Deliver excellent client service by educating patients and managing follow-ups and post-visit needs.
• Communicate with warmth and attentiveness to ensure continuity of care.
• Coordinate with internal and external providers, retrieve medical records and relay wellness recommendations.
• Support team operations by stepping into clinical or administrative roles when needed.
