Qualifications

• High school diploma or GED required.
• Two years’ experience in healthcare settings required.
• BLS certification required.
• Bachelor’s in health or human services preferred; LVN with case management experience preferred; ECM experience preferred.

Responsibilities

• Work with ECM/PCP teams to provide patient care within a comprehensive case management program.
• Assess patient needs, barriers, and goals using a holistic health approach.
• Develop and monitor patient-centric case management plans with providers.
• Use the EMR to track case progress and ensure timely documentation.
• Participate in weekly case reviews and quality improvement projects.
