Job summary

Camino Health Center is seeking a Case Manager to join the Enhanced Care Management (ECM) and PCP teams. The role focuses on integrated medical/behavioral health case management, assessing barriers, and developing patient-centered plans to improve health outcomes.

You will collaborate with ECM, BHS, and providers, document in the EMR, and participate in quality improvement initiatives while upholding Camino's values of Service, Dignity, Justice and Excellence.

Qualifications

• Two years' experience in a healthcare setting (case management preferred).
• BLS certification required.
• Ability to collaborate with PCP/ECM teams and providers.

Responsibilities

• Work with ECM/PCP teams to manage patient cases.
• Perform holistic health assessments including SDOH.
• Develop patient-centric case management plans.
• Monitor progress and adjust goals as needed.
• Maintain accurate EMR documentation.
• Participate in weekly case reviews and quality initiatives.
