Job summary

Advocate Health Care is seeking a Patient Care Assistant to support registered nurses in daily patient care, including bathing, feeding, positioning, and monitoring vital signs. The role requires documentation in the EMR and adherence to infection control policies.

Responsibilities include admission, discharge processes, patient rounds, and ensuring patient comfort and safety with effective communication to the RN and care team. GED or HS diploma and BLS certification are required.

Qualifications

• GED or High School Diploma required.
• Basic computer skills.
• Excellent oral and written communication.
• Excellent organizational skills and ability to set priorities.
• Strong interpersonal skills and adaptability to interruptions.

Responsibilities

• Assists RN with daily patient care including bathing, feeding and mobility.
• Documents vital signs and patient data in EMR.
• Participates in admission, transfer and discharge under RN supervision.
• Performs rounds and communicates patient/family needs to RN.
• Maintains skin integrity and nutritional support under direction of RN.
• Collects specimens and assists with point-of-care testing.
• Follows infection prevention policies and uses sterile/aseptic technique.
• Coordinates tests and treatments with departments and supports hospital systems.
• Maintains positive patient relations and supports a harmonious work environment.
• Meets age-specific care requirements and growth/development considerations.
