AltaMed Health Services is seeking a Patient Care Coordinator (PCC) to serve as a liaison between prospects, patients and AltaMed, helping address concerns and connect patients with services across the care journey.

The PCC will advocate for patients on non-medical issues, assist with enrollment and support families, with a focus on underserved communities and strong bilingual communication where possible. This is a full-time role with opportunities for growth and comprehensive benefits.

Qualifications

• Minimum high school diploma.
• Preferred BS/BA in healthcare or business administration.
• 3 years direct outreach/marketing/sales experience in healthcare preferred.
• 3 years customer service experience preferred.
• CAC certification preferred or to be obtained within 90 days of employment.
• Bilingual Spanish/English or other languages strongly preferred.
• Experience working with underserved populations preferred.

Responsibilities

• The PCC acts as a liaison between prospects, patients and AltaMed and the community.
• Assist patients and family members with enrollment and non-medical concerns.
• Advocate for patients and address issues related to quality of care and access.
