Title: Patient Care Advocate
Location: Reside in OH - Mileage is covered - Cleveland, Columbus, Toledo, Findlay, Athens, Cambridge, Lima, Bowling Green (or nearby to these larger cities)
Duration: 3 Months (Potential to extend and convert to FTE?)
Role and Project Description
Walk me through the day-to-day responsibilities of this role and a description of the project (Outside of Workday JD):
- Coordinate outreach to targeted open HEDIS gaps, reduce barriers and obstacles to care delivery (scheduling appointments, setting up transportation, educating on over-the-counter benefits, educating on healthy rewards available, and educating on preventative care, etc.)
- Describe the performance expectations/metrics for this individual and their team:
- PCA team members will need to communicate with members daily to aid in HEDIS gap closure.
- PCA staff will need to meet/exceed goals for scheduling appointments with members to close focused HEDIS care gaps.
- PCA must be friendly, motivated, and responsible self-starters who can work independently in a remote environment.
- PCA's may go to provider offices within an hour of their home to work with them on scheduling members for care gap closure and need to be available to do this upon request.
- This is an outbound call team.
- Proficiency with Microsoft Excel is beneficial in this role. PCA's must feel comfortable talking with both members and providers.

- Education Requirement: Bachelor's Degree in Healthcare, Public Health, Nursing, psychology, Social Work, Health Administration, or two plus years health field or equivalent work experience
