Job summary

Pawnee Mental Health Services, Inc. is seeking a dedicated Case Manager to assess clients, coordinate benefits, and support recovery across ten Kansas counties. You will work with the treatment team to complete assessments, establish action plans, and ensure continuity of care.

The role requires knowledge of SSI/SSDI processes, Medicaid/Medicare coordination, and strong organizational skills. This is a full-time, on-site position with opportunities to engage with community partners and provide

Qualifications

• Bachelor’s degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education.
• Strong oral and written communication skills using the English language.
• Basic personal computer skills enabling electronic data entry, retrieval, and use.

Responsibilities

• Meet with the client during the comprehensive assessment process to complete vitals and initial testing.
• Assist clients in navigating Medicaid/Medicare benefits and coordinating with case managers and SSA as needed.
• Travel to provide services to clients across ten counties when required.
• Collaborate with Pawnee staff and community partners on benefit acquisition and maintenance.
