Responsibilities

  • Provide curbside consultation to PCPs by phone, electronically, or in-clinic.
  • Offer verbal feedback to PCPs on patient encounters.
  • Conduct scheduled consultations and brief follow-up visits (6–8 sessions).
  • Screen for common conditions, assess, and intervene for chronic disease management programs.
  • Diagnose and treat mental and behavioral disorders; conduct risk assessments as needed.
  • Deliver psychoeducation during individual and group visits.
  • Adapt educational materials for literacy and cultural appropriateness under oversight.
  • Use evidence-based practices in interventions.
  • Prepare and document assessment findings, interventions, and recommendations within seven days.
  • Maintain timely paperwork to comply with administrative regulations.
  • Support medication protocols and collaborate with PCPs and other BH providers.
  • Attend CHWP meetings, including staff, PCP, and team meetings as requested.
  • Assist in clinical supervision of CHWP BH staff for licensure requirements.
  • Ensure patient confidentiality and participate in clinical oversight of staff.
  • Conduct periodic record reviews of clinical notes and treatment goals.

Integrated Health Experience – Behavioral Health Therapist (LISW, LISW‑S, LPCC, LPCC‑S)

Job Summary

This full‑time or part‑time position is located in Urbana, OH, with additional sites in Bellefontaine and West Liberty. The Behavioral Health Provider works within a team‑based care model, applying social work methods to diagnose, prevent, and treat psychosocial dysfunction for individuals, families, and groups. Responsibilities include screening, assessment, goal setting, and intervention; consulting with primary care providers; and providing brief, evidence‑based treatment during in‑clinic or telehealth sessions.

Qualifications and Requirements
  • Education – Master’s degree in Social Work, Counseling, or related field.
  • Certifications and Licenses
    • Valid Ohio driver’s license.
    • Ohio Licensed Social Worker, Licensed Independent Social Worker, Licensed Independent Social Worker Supervisor, Licensed Professional Counselor, Licensed Professional Clinical Counselor, or Licensed Professional Clinical Counselor Supervisor.
  • Professional Experience
    • Expertise in behavioral health, substance use practices, protocols, and best practices.
    • Success in affecting overall clinical operations.
    • Knowledge of accreditation processes and all federal, state, and local regulations for community health centers.
    • Ability to manage multiple responsibilities and emergency situations.
    • Proficiency in office management and use of computer software packages.
Physical/Environmental
  • Occasional lifting of 40–50 lbs. and pushing of 5–20 lbs.
  • Approximately 80% sitting, standing, and walking.
  • Medium to high manual dexterity required.
  • Bending and reaching about 20% of the time.
  • Normal accessibility and mobility throughout the region required.
  • Field may involve overtime/extended work hours.
Working Hours

Full‑time (approximately 40 hours/week); part‑time as agreed. Hours typically Monday–Friday, with occasional Saturday or after‑business hours. Hybrid on‑site and telehealth shifts available.

Wages

Starting wages negotiable based on CHWP pay policies, education, experience, and location. Reflects national and state compensation averages.

Introductory Period

90‑day probationary period; successful completion does not alter at‑will status.
