Overview
CHWP is an integrated primary care community health center that utilizes a team-based care model, sharing patient‑care responsibilities among members of a team. The Team Physician provides direct clinical care within the scope of a primary care physician and functions as a Team Leader overseeing the full operations of a team‑care model, delivering whole‑person, high‑quality care. The physician collaborates with nurse practitioners through collaborative care agreements and supports CHWP’s quality initiatives and the requirements of a Federally Qualified Health Center and related certification programs such as HRSA, PCMH, and Joint Commission.
Primary Accountabilities

- Directly improve patient well‑being through accurate, timely diagnosis and delivery of high‑quality medical care.
- Provide leadership and direction to all team members and ancillary staff to meet or exceed measurable standards of care, including overall clinical, quality, and productivity goals.
- Ensure accuracy, efficiency, and confidentiality in patient charting, recordkeeping, and administrative functions.
- Operational Excellence
- Ensure all medical treatment is consistent with regulatory, accreditation, and professional standards, including clinical policies and quality initiatives.
- Maintain adherence to contracted protocols and procedures related to the delivery of medical services.
- Lead the full operations of the team‑care model, engaging all team members in whole‑person, integrated primary care delivery.
- Relationship Management
- Develop and maintain positive relationships with patients, families, and other CHWP staff.
- Encourage patient adherence to effective care plans and deliver services in a caring, professional manner.
- Serve as a mentor and coach to foster a cohesive approach to integrated care services.
- Professionalism & Stewardship
- Act in a highly professional manner in performance, conduct, and communication.
- Ensure compliance with CHWP policies, procedures, and quality initiatives.
- Practice within the scope of a nurse practitioner for primary care or related specialty.
- Serve CHWP’s mission by providing quality, whole‑person, patient‑centered care to all community members.

- Participate in stewardship opportunities at least twice a year and maintain HIPAA compliance.
Primary Tasks and Duties
- Conduct comprehensive physical assessments, evaluate, diagnose, and treat new and existing patient conditions.
- Order and interpret laboratory and diagnostic studies.
- Perform medical procedures as credentialed.
- Prescribe medications and refer patients for specialized consultation.
- Write and dictate progress notes indicating patient status and treatment performed.
- Conduct follow‑up patient care and provide health education to patients and families.
- Supervise and coordinate activities of patient care and team care.
- Provide periodic reports to management on team‑care model progress.
- Contribute to teamwork goals and encourage maximum participation from all members.
- Teach and train in illness prevention and community health education.
- Perform emergency life‑saving procedures (e.g., cardiac arrest, respiratory arrest, massive hemorrhage).
- Serve as a front‑line provider in emergency disaster services.
Work Environment
The work environment is a standard medical office setting. Reasonable accommodations may be made for individuals with disabilities. Work may include occasional weekend and evening hours and outpatient call on an equal basis from home.
Work Hours: Full‑time is typically a 50–60‑hour week; part‑time is negotiated. Office hours are usually 8:00am–6:00pm, Monday through Friday, with some Saturdays and after‑business hours as required. Meetings and travel are part of the position.
Wages: Starting wages for the physician are negotiable based on CHWP pay policies and factors such as education, experience, and regional compensation benchmarks. Compensation details are described in the Employment Agreement.
Introductory Period: Identified in the Employment Agreement.
Credentialing Process: The physician will complete the CHWP credentialing process prior to hire and will participate in all payer contracts (Managed Care, Medicaid Managed Care, and others) engaged by CHWP.

Qualifications and Key Competencies
- Education: Graduate of an ACGME accredited residency program in the specialty area; Award of Medical Doctor (MD) or Doctor of Osteopathic (DO) degree.
- Certificates and Licenses:
- Valid Ohio Driver's License
- Board certification within specialty (State Medical Board of Ohio)
- Basic Life Support (BCLS) or Advanced Cardiac Life Support (ACLS) certification
- Professional:
- Excellent leadership experience in a critical role.
- Strong team‑building skills.
- Expertise in relevant medical practices, protocols, trends, and best practices.
- Experience with accreditation processes and all federal, state, and local regulations for community health centers.
- Ability to manage multiple responsibilities and emergency situations.
- Proficiency with office management, computer use, and electronic health record systems.
- Physical/Environmental:
- Occasional lifting of 40–50lbs and pushing of 5–20lbs.
- Ability to sit, stand, and walk approximately 80% of the time.
- Moderate manual dexterity and flexibility for bending and reaching circa 20% of the time.
- Normal mobility throughout the region and willingness for extended work hours.
