Job summary

St. Petersburg Free Clinic in Saint Petersburg, FL is seeking a Health Access Coordinator to coordinate patient referrals and care transitions within the SPFC Health Center and with external partners.

You will work with staff, Resource Navigators, Welcome Service Coordinators, and SPFC partners to ensure timely, appropriate access to services. Familiarity with EMR, referral tracking, and compassionate patient-centered care is essential.

Qualifications

• Strong relationship-building with internal and external stakeholders.
• Proficiency with EHR systems and referral tracking tools.
• Excellent phone and email communication; handling sensitive topics.
• Bilingual skills are highly preferred.

Responsibilities

• Collect patient referral needs from clinicians and care team.
• Verify referral information and urgency.
• Prioritize referrals based on clinical urgency and barriers to access.
• Route referrals to correct specialty/service line.
• Schedule and track referrals and follow ups from submission to completion.
• Refer patients to SPFC Navigators for social/non-medical needs.
• Educate patients on next steps and referrals.
• Maintain accurate EMR notes and communications.
• Track barriers and progress toward patient care goals.
• Collaborate with Navigation Department and SPFC staff to meet patient needs.
