Job summary

Whitney M. Young Jr. Health Center is seeking a Patient Access Coordinator to support the Practice Operations Team. You will prepare patient charts, focus on gaps in care, and assist with registrations and referrals across WYH locations.

You will work with site staff to review visit planning workflows, document in the EHR, and ensure compliance with HIPAA and patient confidentiality while delivering exemplary customer service.

Qualifications

• Associates degree in health field and two years’ experience in healthcare, with 3–5 years acceptable in lieu of degree.
• Strong written and verbal communication skills and ability to solve problems in a patient-facing role.
• Proficiency with Microsoft Office, especially Excel, and the ability to maintain confidentiality.

Responsibilities

• Prepare patient charts for future appointments with focus on gaps in care and standards of care.
• Coordinate referrals and maintain documentation in the EHR and with external providers.
• Identify and link patients with community resources to support referrals.
• Ensure timely registration and accurate patient demographics and insurance information.
• Maintain HIPAA compliance and high-quality customer service in interactions with patients and staff.
