Job summary

Native American Community in Minneapolis, MN is seeking a Patient Support Coordinator to enhance patient engagement by assisting with digital health tools and health insurance changes. You will support portal sign-ups, troubleshoot access issues, and coordinate with care teams to minimize coverage disruptions.

You'll educate patients on Medicaid eligibility changes, enrollments, notices, and deadlines, while ensuring privacy and HIPAA compliance.

Qualifications

• High school diploma or GED required; Associate’s degree or higher preferred.
• 1-2 years experience in healthcare, IT support, or customer service, ideally patient-facing.
• Proficiency with EHR systems; Epic/MyChart strongly preferred.

Responsibilities

• Assist patients with MyChart sign-ups and health insurance assistance.
• Educate patients about Medicaid eligibility changes, renewals, verification and enrollment.
• Explain insurance notices in patient-friendly language and conduct outreach to those at risk of losing coverage.
• Help patients understand deadlines and required actions while ensuring privacy compliance (HIPAA).
• Support IT and data teams with Epic updates and data collection related to health insurance workflows.
• Educate patients on portal features (appointments, test results, messaging, medications).
• Collaborate with clinical/admin teams to support population health initiatives and portal use for patient communication.
