Qualifications

• Must read, write, and speak fluent English.
• High School Diploma or GED required; associate or bachelor’s degree preferred.
• Healthcare experience preferred; post-acute care a plus.
• Strong data entry and attention to detail.

Responsibilities

• Serve as key team member of the new Central Access and Authorizations Team (CAAT), focused exclusively on intake of referrals and building the initial case for patient admission.
• Initiate patient referrals by creating patient accounts in appropriate referral management platform.
• Enter demographic information into referral platform and Meditech for admission.
• Coordinate bed assignments and admission paperwork with facility leadership.
• Ensure insurance verification and authorization are complete.
• Maintain the pending referral log and reconcile referral and patient accounts.
• Support ongoing education and process improvements within the team.
