Job summary

Volunteer Behavioral Health Care System (VBHCS) is seeking a dedicated PATH Coordinator for Putnam County. This full‑time, field‑based role conducts street and community outreach to engage individuals experiencing homelessness and provides comprehensive case management toward housing stability and recovery goals.

The position requires a bachelor's degree in a related field, experience with adults who have mental health needs, and knowledge of psychotropic medications.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or a related field.
• Experience in community settings with adults with severe mental illness.
• Knowledge of psychotropic medications and side effects; homelessness experience is a plus.

Responsibilities

• Conduct field outreach to identify and enroll eligible participants.
• Maintain an active caseload and develop individualized service plans.
• Assist clients in accessing shelters, housing resources, and services.
• Collaborate with CoC and community agencies; participate in case conferences.
• Complete daily updates and documentation to meet grant objectives.
