Responsibilities include data entry into HMIS, assisting clients with entitlements (Medicaid, SSI/SSDI, SNAP), and driving to meet clients' needs. Bachelor’s degree required; OBHP certification within 3 weeks of hire.

Qualifications

• Bachelor’s Degree is required.
• Experience in mental health or related field preferred.
• Community outreach experience is a plus.

Responsibilities

• Provides documentation of interventions per department standards.
• Drives and transports clients as needed for PATH related business.
• Data entry into HMIS.
• Apply for social security benefits through SOAR.
