Job summary

Porter-Starke Services, Inc. is seeking a PATH Case Manager to outreach to individuals who are homeless or at imminent risk, connecting them with housing, mental health services, Medicaid, SNAP, and SOAR.

This full-time role documents interventions, coordinates with partners, and assists clients in obtaining entitlements and benefits.

You will collaborate with a multidisciplinary team, participate in trainings, and advocate for vulnerable populations with compassionate, client-centered care.

Qualifications

• Bachelor’s degree required.
• Experience in mental health or related discipline preferred.
• Community outreach work with vulnerable populations is a plus.
• OBHP training/completion required within 3 weeks of hire.

Responsibilities

• Provides street outreach to clients.
• Coordinates with community partners to connect services.
• Documents interventions per department standards.
• Drives/transports clients for PATH-related business.
• Assists clients in obtaining entitlements such as Medicaid and SSI/SSDI.
• Performs data entry into HMIS and related systems.
