Job summary

Cumberland County Guidance Center is seeking a dedicated Case Manager / Housing Specialist to join our PATH program in New Jersey. The role focuses on outreach, housing navigation, and recovery support for individuals experiencing homelessness and serious mental illness.

The successful candidate will develop IRPs, coordinate services, assist with housing placements, and connect clients to healthcare, benefits, and community resources. Strong advocacy and collaboration skills are essential.

Qualifications

• Bachelor's degree in Social Work or related Human Services field.
• Minimum two years of experience in case management, social services, behavioral health, mental health services, homeless services or community outreach.
• Knowledge of housing resources, public benefit programs, and community agencies.
• Valid driver's license with acceptable driving record.

Responsibilities

• Conduct community outreach and engagement with individuals experiencing homelessness or housing instability.
• Complete assessments, screenings, and service planning activities.
• Develop and implement Individual Recovery Plans and person-centered care plans.
• Provide ongoing case management and care coordination services.
• Assist clients with securing permanent, supportive, and emergency housing resources.
• Connect individuals to mental health, medical, and community support services.
• Advocate for entitlement programs and public benefits; navigate community resources.
• Collaborate with treatment providers, families, landlords, and community partners.
• Deliver recovery-oriented interventions using Motivational Interviewing and evidence-based strategies.
• Document services and ensure compliance with program requirements.
• Participate in multidisciplinary team meetings and community partnerships.
