Job summary

Community Health and Counseling Services (CHCS) is seeking a Path Navigator to support the PATH program in Maine. The role involves outreach, engagement and service coordination for adults who are homeless and have a mental illness or co-occurring disorders, with a focus on linking individuals to needed resources.

The Navigator will assess needs, plan interventions, monitor progress and may participate in street outreach to connect with individuals in shelters, outdoors, or community locations.

Qualifications

• Experience working with homeless persons with disabilities.
• Valid State of Maine driver's license and automobile liability coverage.
• PATH Navigator duties include outreach, engagement, assessment, planning and linking to resources.

Responsibilities

• Participate in street outreach and engage people who are homeless in various settings.
• Identify eligible individuals and assess needs for PATH program supports.
• Plan, link to resources, and monitor services for adults who are homeless with mental illness or co-occurring conditions.
