Job summary

WelbeHealth, LLC. is seeking a Participant Experience Liaison to ensure smooth onboarding to the PACE program and ongoing support throughout each participant's WelbeHealth journey.

You will coordinate with the IDT, external providers, families, and participants to deliver high-quality care, address concerns, and drive satisfaction. A background in senior care and strong communication are essential.

Qualifications

• Excellent communication with seniors, families, and IDT members.
• Strong organizational and customer service abilities in senior care settings.
• Ability to present information to diverse groups and leadership.
• Capable of working in a fast-changing environment and making sound decisions.

Responsibilities

• Act as a liaison and maintain open communication with IDT and external providers.
• Assist management with participant experience and service recovery projects.
• Facilitate communication among service providers, families, participants, and caregivers.
• Provide customer service calls to ensure satisfaction with IDT and specialty care.
• Support the grievance process and work toward resolution with IDT and participants.
