St. Paul’s Senior Services is seeking a Participant Care Coordinator (PCC) to schedule participant appointments across medical, dental, therapy, and transportation services.

The role ensures timely execution of ordered appointments, supports the interdisciplinary care team, and maintains clear communication with participants, caregivers, and providers. • High School Diploma or equivalent required.
• At least 1 year of administrative support experience.

Responsabilidades

• Schedule appointments within regulatory timeframes.
• Document information in medical records and enter data across platforms.
• Coordinate follow-up appointments with IDT and records department.
• Prepare packets for specialty visits.
• Attend daily IDT meetings.
• Communicate appointment details to participants and caregivers.
• Answer department inbound calls via call queue.
• Maintain scheduling KPIs including turnaround times and queue metrics.
• Maintain records and ensure privacy compliance.
• Perform other duties as assigned.
