Job summary

PACE Southeast Michigan is looking for a Participant Advocate to ensure the smooth onboarding of new enrollees into the PACE SEMI Program. Responsibilities include escorting new participants during their initial visits, providing education on PACE services, and advocating on behalf of participants.

The ideal candidate should have an Associate's degree and experience with the elderly population, demonstrating excellent communication and organizational skills. This position plays a crucial role in facilitating the successful transition of participants into our program.

Qualifications

• Minimum of one year's experience with a frail or elderly population.
• Knowledge of health, social, and behavioral problems of the frail elderly.

Responsibilities

• Escort new intakes through their initial visit.
• Represent the Intake and Enrollment department in respective centers.
• Facilitate pre-enrollment evaluations with specialists.
