Requirements include an LPN/LVN license, HS diploma or GED, 2+ years LPN/LVN experience (SNF/LTACH preferred), and BLS. Willingness to assist vein clinic and documentation duties is expected.

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program.
• Current LPN/LVN license in the practicing state and maintain throughout employment.
• Minimum 2 years LPN/LVN experience; SNF or LTACH experience preferred.

Responsibilities

• Provide wound-related and basic nursing care under RN/Provider supervision.
• Assist in vein clinic after Vein Clinic training.
• May assist with intake and discharge and case management under RN.
• Gather patient information and prepare equipment for RN or provider.
• Report care quality issues to theClinical Nurse Manager and communicate with the team.
• Possibly perform hyperbaric therapy duties after training and may act as scribe as needed.
