Job summary

Spectrum Retirement Communities is seeking a Wellness Nurse for a part-time role with a 7am-3pm shift, every other weekend, in Libertyville, IL. The nurse will monitor residents' health, support staff, and ensure regulatory compliance while delivering compassionate care.

The role requires a practical nursing credential, an active LPN or RN license, and experience in geriatric assessment. Collaboration with community nurses and providers is essential to deliver high-quality resident care.

Qualifications

• Knowledge of state regulations, policies and procedures for compliance.
• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Three years of experience in assessment and geriatric nursing preferred.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs.
• Ensure care is delivered as indicated in resident care plans.
• Medication and treatment administration per physicians’ orders.
• Coordinate care and services with outside providers including resident physicians.
• Document and report care provided and resident observations including significant changes.
• Maintain knowledge of state regulations, policies, and procedures.
