Job summary

Ohana Pacific Health is hiring a part-time Speech-Language Pathologist to develop and implement a progressive home health rehabilitation program. The role includes case management, supervision of Home Health Aides, and assessments of speech, language, cognition, and swallowing.

Compliance with Hawaii licensure and CPR requirements is required. Responsibilities include evaluating patients, coordinating treatments, re-assessing as needed, and collaborating with interdisciplinary teams to meet

Qualifications

Master's degree in Speech-Language Pathology from ASHA-accredited program.
Hawaii state license to practice as a Speech-Language Pathologist.
Current Basic Life Support CPR certification (AHA).

Responsibilities

Develop and implement a progressive rehabilitation program for patients in their home.
Serve as case manager and supervise Home Health Aides as appropriate.
Screen, diagnose, and treat or consult on disorders of communication, cognition, and swallowing.
Perform patient evaluations, admissions, re-assessments, and discharges based on criteria.
Develop and supervise Home Health Aide care plans and coordinate interdisciplinary referrals.
