Job summary

Ohana Pacific Health is seeking a Part-time Speech-Language Pathologist to develop and implement a progressive home-based rehabilitation program. You will serve as the case manager and supervise home health aides, evaluating and treating communication, cognition, and swallowing disorders for patients in their homes.

The role requires a Master's degree in Speech-Language Pathology, Hawaii licensure, 2-step TB clearance, and CPR certification. Previous home health experience is preferred.

Qualifications

• Master's degree in Speech-Language Pathology from ASHA-accredited program.
• Hawaii state license to practice as a Speech-Language Pathologist.
• Current Basic Life Support (BLS) CPR certification.
• Two-step TB clearance.
• Experience in Home Health environment preferred.

Responsibilities

• Develop and implement a home-based rehabilitation program for patients.
• Serve as case manager and supervise Home Health Aides as appropriate.
• Screen, diagnose, and treat or provide consultations for disorders of communication, cognition, and swallowing.
• Perform ongoing re-assessments and discharge patients when goals are met or criteria no longer met.
• Develop Home Health Aide care plans and delegate duties per agency policy.
