Job summary

Ohana Pacific Health is seeking a part-time Speech Language Pathologist to develop and implement a home health rehabilitation program, serving patients in their homes across Hawaii. You will act as case manager and supervise home health aides as needed, assessing and treating communication, cognition and swallowing disorders.

Responsibilities include evaluation, admission decisions, diagnostic testing, planning and re-assessment, and discharge when goals are met.

Qualifications

• Master's degree in Speech/Language Pathology from an ASHA-accredited program.
• Current Hawaii license to practice as a Speech Language Pathologist.
• Two-step TB clearance and ongoing health screening.
• Active Basic Life Support (CPR) certification.
• Experience in home health settings preferred.

Responsibilities

• Evaluate assigned patients and admit them based on agency criteria.
• Determine factors for speech, language, cognition and swallowing impairments and plan treatment.
• Provide diagnostic testing, plan implementation, re-assessments and discharge when goals are met.
• Develop Home Health Aide (HHA) care plans and supervise HHAs as appropriate.
• Manage case coordination, referrals, and interdisciplinary collaboration.
• Deliver direct patient care and educate patients, families, and caregivers.
