Job summary

Ohana Pacific Health is seeking a Speech Language Pathologist to develop and implement a progressive rehabilitation program for patients in their home, ensuring care aligns with professional standards and patient needs. The role includes case management and supervising Home Health Aides when appropriate.

You will screen, diagnose, and treat communication, cognitive, and swallowing disorders, evaluate patients, admit based on agency criteria, and coordinate interdisciplinary referrals to support

Qualifications

• Master’s degree in Speech/Language Pathology from ASHA-accredited program
• Hawaii state license to practice as a Speech Language Pathologist
• Current Basic Life Support CPR certification (AHA)
• 2-step TB clearance
• Experience in Home Health preferred

Responsibilities

• Develops and implements a progressive rehabilitation program for home patients; serves as case manager
• Supervises Home Health Aides when appropriate; delegates duties per policy
• Screens, diagnoses, and treats or provides professional consultations for speech, language, cognition, and swallowing disorders
• Performs patient care duties and educates patients, families, and caregivers in treatment plans
