The Informed SLP, LLC in Washington is seeking a part-time Speech-Language Pathologist (SLP) to provide evaluated services with a focus on improving speech and language function. The role involves working within the home health plan, delivering therapy, counseling caregivers, and drafting clinical notes.

You will collaborate with physicians and other health care professionals, identify needed equipment, and participate in care conferences to ensure well-coordinated care for patients in the home

Qualifications

• Conducts appropriate evaluations and identifies equipment needs relative to speech/language function.
• Assists in the ongoing interdisciplinary assessment and development of the plan of care.
• Administers speech therapy program utilizing specialized techniques and equipment.
• Recommends communication devices/aids as indicated.

Responsibilities

• Assumes responsibility for ongoing interdisciplinary assessment and plan of care in partnership with patient, representative, and caregiver(s).
• Provides services ordered by the physician as indicated in the plan of care.
• Provides patient, caregiver, and family counseling and education.
• Prepares clinical notes and communicates with the physician and other health care practitioners.
• Identifies functional speech defects and establishes a plan of care to improve the patient’s function.
• Participates in case conferences to ensure coordinated and comprehensive care.
