Phoenix Home Care in Lebanon, MO is seeking a licensed Speech-Language Pathologist for part-time home health visits. You will provide direct therapy to patients, perform assessments, and develop plans of care in collaboration with physicians and the team.

Qualifications

• Master's degree in Speech-language pathology required.
• Current license in the state of practice.
• One year clinical experience preferred in acute care or rehabilitation.

Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician.
• Documents care accurately, timely, and legibly.
• Collaborates with team to review and update the Plan of Care.
• Supports agency financial and productivity goals.
