Job summary

Phoenix Home Care & Hospice is seeking a Part-Time Speech-Language Pathologist to assess, deliver, supervise and evaluate speech therapy services for agency patients in Columbia, MO and surrounding areas.

Responsibilities include direct therapy, documentation, and collaboration with the care team, with a focus on quality outcomes. Requires a Master’s degree in Speech-Language Pathology and current Missouri license; travel may be required.

Qualifications

• Master’s degree in Speech-language pathology and current license in Missouri.
• Prefer one year of clinical experience, preferably in home health or rehabilitation setting.

Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician and in accordance with the Plan of Care.
• Provides documentation in accordance with agency standards of all care and services provided to patients.
• Communicates and collaborates with team members when reviewing, revising, and updating the patient’s Plan of Care.
• Performs duties in alignment with agency financial and productivity goals and policy.

Education

Master’s degree in Speech-language pathology

Current license in the state of practice (Missouri)
