Job summary

Total Care Therapy is seeking a Speech-Language Pathologist to evaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in older adults within assisted living and independent living communities in Shaker Heights, OH. The role involves developing individualized treatment plans and delivering evidence-based therapy.

This part-time position offers per-visit compensation ($60-$70), flexible scheduling (Mon-Fri, part-time; weekends optional), and a therapist-owned

Qualifications

• Current State Speech-Language Pathologist (SLP) License required in the state of practice.
• CCC-SLP preferred or eligibility if applicable.
• Current CPR/BLS Certification required.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized speech therapy plans of care.
• Assess and treat speech, language, voice, cognitive-communication, and swallowing disorders.
• Perform dysphagia evaluations and implement appropriate swallowing interventions.
• Reassess patient progress and adjust treatment plans as needed.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on communication strategies and swallowing precautions.
• Collaborate with physicians, nursing staff, dietitians, and interdisciplinary team members to ensure coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering evidence-based therapy services.
