Functional Pathways is seeking a Speech Language Pathologist to screen, evaluate, and update care plans for residents in a facility setting. The role operates under physician supervision with a focus on maximizing independence and safe discharge planning.

Responsibilities include conducting quarterly screenings, developing treatment plans, reassessing progress every 30 days, educating staff and families, coordinating equipment, and maintaining thorough documentation.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• ASHA CCC Certification and active state license.
• Experience in geriatric care and rehabilitation.
• Excellent organizational, written and verbal communication skills.

Responsibilities

• Screen and evaluate residents; develop and revise plans of care under physician supervision.
• Create treatment plans, monitor progress, and reassess every 30 days.
• Provide discharge planning to maximize independence in the least restrictive environment.
• Educate staff and families; coordinate equipment and supplies for therapy.
• Document evaluations, plans, progress notes, and discharge summaries.
• Attend facility meetings and ongoing continuing education.
