Job summary

Colfax of Cascadia LLC is seeking a compassionate Part-Time Speech Language Pathologist (SLP) to support residents in a sub-acute and skilled nursing setting. The role involves evaluating and treating speech, language, cognitive-communication, and swallowing disorders while coordinating with an interdisciplinary team.

You will develop individualized plans, deliver direct therapy, and maintain thorough documentation to monitor progress and outcomes.

Qualifications

• Bachelor’s degree in Speech Language Pathology required.
• Current license in Speech Language Therapy as required by state law.
• CPR certification required.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Screen, assess, and evaluate residents/patients with speech, language, cognitive-communication, voice, swallowing, and fluency disorders.
• Develop individualized care plans based on patient needs, physician directives, and rehabilitation goals.
• Provide direct speech therapy services to address communication, cognitive, and swallowing impairments.
• Plan and conduct rehabilitative treatment programs to improve functional outcomes and communicative effectiveness.
• Maintain accurate clinical documentation including evaluations, treatment notes, progress reports, and summaries.
• Collaborate with physicians, nurses, and interdisciplinary team to coordinate patient-centered care.
