Job summary

Lone Tree Post Acute is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a part-time basis.

You will evaluate residents for speech, language, cognitive, and swallowing disorders; develop and implement treatment plans; provide skilled therapy; monitor progress; collaborate with the interdisciplinary team; educate residents and families; and keep accurate documentation.

Qualifications

• Current California Speech-Language Pathologist license.
• Master’s degree in Speech-Language Pathology from an accredited program.
• ASHA CCC-SLP certification preferred.

Responsibilities

• Evaluate residents for speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Provide skilled speech-language therapy based on residents’ needs.
• Monitor progress and adjust treatment plans as appropriate.
• Collaborate with nursing, therapy, and interdisciplinary team members.
• Educate residents, families, and caregivers regarding treatment recommendations.
• Maintain accurate and timely clinical documentation.
