Job summary

Masonic Village at Warminster, part of Masonic Villages of Pennsylvania, is hiring a Social Worker - Long Term Care for a part-time, daytime role with no weekend work. You will support admissions, plan and evaluate psychosocial care, and provide counseling as needed to residents and families.

Responsibilities include discharge planning, documentation, confidentiality, and collaboration with the care team. Requires a BSW and long-term care experience; salary ranges from $24 to $35 per hour.

Qualifications

• Bachelor's degree in Social Work or related health field.
• Empathy-driven interviewing skills.
• Clear, compassionate communication with referrals, staff and families.
• Computer literacy including word processing; familiarity with MDS.
• Experience with adults/geriatrics; long-term care experience required.

Responsibilities

• Assist Admissions Director in the inquiry and admission process, conduct interviews, schedule admissions, and coordinate with other departments for smooth entry.
• Perform social service assessments and develop plans of care with ongoing evaluation of resident needs.
• Provide social work counseling/therapy when indicated for patients and families.
• Maintain strict confidentiality of sensitive information in the clinical record.
• Document resident adjustment, progress toward goals, and significant behaviors; maintain logs and bed hold letters when needed.
• Conduct confidential interviews in suspected abuse cases as part of safeguarding residents.
• Coordinate discharge planning and resources (home health, Life Line, Meals-on-Wheels, equipment).
• Contribute to interdisciplinary care planning and participate in care conferences; communicate needs to attending physician.
• Attend mandatory in-services and continuing education to stay current in practice.
