Job summary

Masonic Villages of Pennsylvania seeks a Social Worker for Long Term Care at Warminster Campus. This part-time role covers days with no weekends, focusing on planning, implementing care plans and social services in compliance with regulations.

The successful candidate will assist admissions, conduct psychosocial assessments, provide counseling, document progress, and collaborate with the interdisciplinary team to meet resident goals and ensure smooth transitions.

Qualifications

• Bachelor's degree in Social Work (BSW) or other health-related degree.
• Interviewing skills and ability to empathize with residents and families.
• Strong communication with referral sources, medical staff, families, residents, and staff.
• Computer literacy including word processing and MDS completion.
• Experience working with adults/geriatrics preferred; long-term care experience required.

Responsibilities

• Assists Admissions Director with admissions and inquiries; guides tours and coordinates paperwork.
• Performs psychosocial assessments and plans of care; evaluates resident needs.
• Provides counseling/therapy for residents and families when indicated.
• Maintains confidentiality of sensitive information.
• Documents resident progress and updates records quarterly or as needed.
• Conducts confidential interviews in suspected abuse investigations.
• Coordinates discharge planning and community resource referrals.
• Contributes to interdisciplinary care planning and resident goal setting.
• Participates in mandatory trainings and ongoing professional development.
