Job summary

Qualifications

• Master of Social Work or Doctorate in social work.
• Minimum one year of social work experience in healthcare.
• Licensure as LCSW in Virginia and CPR certification required.

Responsibilities

• Assist patients and families with financial, social and emotional concerns.
• Provide social work services per the plan of care.
• Communicate patient status changes to the care team in a timely manner.
• Educate patient and family on interventions and treatment plan.
• Perform care planning, transfers and discharge functions.
• Identify risks in the patient’s environment and coordinate mitigation.
