• CCC-SLP is preferred and may be required by state or contract.
• CPR current if applicable.
• TB questionnaire, PPD or chest x-ray if applicable.

• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Performs student evaluations in the school setting.
• Provides and documents therapy services per individual plan of care and physician orders.
• Maintains documentation in accordance with school and contract requirements.

Education

Active SLP Licensure in the state of assignment
