Job summary

Qualifications

• Current license to practice as a Registered Nurse in New Jersey.
• BSN with 2 years of professional experience in community health; or RN with 3 years professional experience in community health.
• At least one year experience in a DHS funded residential setting preferred.
• Driver's license valid in New Jersey.
• Exceptional customer service and communication skills.
• Strong management and leadership skills.
• Background checks including DHS and related screenings as required.

Responsibilities

• Oversees maintenance of residents' clinical records and licensing compliance.
• Provide oversight of Registered Dietitian Nutritionist.
• Educate and evaluate staff; promote ongoing professional development.
• Conduct monthly and quarterly audits of medical and medication files.
• Collaborate with Residential Managers to correct medical records.
• Assist management with coaching and refresher training sessions.
• Maintain communication with clients, physicians, pharmacy, and therapists.
• Manage transitional care, doctor appointments, and resident care with the Residential Manager.
• Schedule medical appointments and procedures; attend medical appointments as needed.
• Collaborate with care team to carry out treatment plans with PCPs and specialists.
• Participate in IDT and Annual ISP meetings; complete incident reports and QI activities.
• Schedule one-on-one meetings with the Registered Dietitian Nutritionist; conduct annual performance reviews.
• Participate in Jfed leadership meetings and Weinberg Commons meetings as requested.
• Follow Jewish Federation policies and procedures.
