Job summary

Twenty80 llc is seeking a part-time Nurse Supervisor (RN) to oversee clinical operations in a home health and assisted living setting in New Jersey. The role blends direct patient care with care coordination and leadership to ensure quality outcomes.

The ideal candidate holds an active NJ RN license, a BSN, and at least 2 years of RN experience in community/home health. This position offers 20 hours per week onsite with a competitive salary.

Qualifications

• Active RN license in New Jersey in good standing.
• BSN or related healthcare degree from an accredited institution.
• Minimum of 2 years professional RN experience, preferably in community/home health.
• Experience providing nursing services in patient homes and assisted living settings.
• Strong clinical assessment and care coordination skills.
• Excellent English communication, both verbal and written.
• Valid driver’s license and active auto insurance.
• Ability to perform physical requirements including lifting, bending, and standing.

Responsibilities

• Assess and document patients' physical, psychosocial, and educational needs while providing preventative, supportive, and restorative nursing care.
• Administer and document prescribed medications and clinical treatments according to care plans and physician orders.
• Coordinate patient appointments, therapies, and multidisciplinary care activities to support continuity of care.
• Monitor patient conditions and communicate significant changes to the Director of Nursing and appropriate healthcare providers.
• Provide clinical guidance and support to patients, families, and care teams.
• Ensure compliance with infection control procedures, standard precautions, HIPAA regulations, and safety standards.
• Maintain accurate and timely patient documentation, clinical records, and required reports.
• Demonstrate sound clinical judgment and participate in quality improvement initiatives.
• Support daily nursing operations and collaborate with leadership to improve care delivery.
