Akron Children's is seeking a part‑time School Based Health Care Coordinator to manage health services for students in Akron‑area schools. The role requires RN licensure in Ohio, ADN with BSN within 5 years, and pediatric health experience. Epic EMR proficiency and BLS CPR are required.

The coordinator will partner with school nurses, families, and external providers to identify health barriers, coordinate care, and ensure students access needed services. This is onsite, 0.7 FTE, 35 hours/week.

Qualificações

• Pediatrics experience required with pediatric healthcare background.
• RN license in Ohio is required; BSN within five years if ADN used.
• BLS CPR certification required.
• Associate degree in nursing required; BSN preferred or required within five years.
• Minimum 2 years of pediatric healthcare experience; school-based or community health setting preferred.

Responsabilidades

• Coordinate and manage health services for students within the school system.
• Identify, assess, and address health barriers to student success.
• Collaborate with students, families, school staff, and primary care providers.
• Bridge gaps to health care and coordinate referrals to medical, mental health, and social services.
• Maintain confidential health records and prepare health trend reports.
