Laconia Nursing Home is seeking an experienced RN MDS Coordinator for a part-time role. The primary responsibility is coordinating the RAI process, ensuring timeliness and completeness of MDS, CAA’s, and interdisciplinary care plans.

The ideal candidate holds a Nursing Degree, an active RN or LPN license with CPR, and at least 2 years of long-term care MDS experience. The role offers growth opportunities and competitive benefits.

Qualifications

• RN/LPN with Nursing Degree and CPR certification.
• Minimum 2 years' experience as MDS Coordinator in Long Term Care.
• Knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.

Responsibilities

• Coordinate and monitor MDS assessments for timeliness.
• Input MDS data and transmit to state with validation receipt.
• Schedule and facilitate care plan conferences, QM meetings.
• Oversee the care planning process.
