Job summary

Laconia Nursing Home seeks an experienced full-time RN MDS Coordinator to supervise MDS assessments, ensure timely submissions to the state, and lead care plan conferences in a long-term care setting.

• At least 2 years’ experience in Long Term Care as an MDS Coordinator.

Responsibilities

• Inputting of MDS assessments, state transmission, and receipt validation.
• Schedules and facilitates care plan conferences; leads monthly QM meetings.
• Manages the Care Planning Process.

• Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility
• Ability to organize and prioritize.
