Mission Valley Post Acute is currently hiring a part‑time MDS Coordinator RN to join our clinical leadership team. The role focuses on coordinating the MDS process, ensuring accurate documentation, timely assessments, and compliance with regulatory and reimbursement requirements.

Ideal candidates are experienced RNs with strong organization, attention to detail, and ability to collaborate across interdisciplinary teams to maintain high‑quality resident care.

Qualifications

• Current Registered Nurse (RN) license in California is required.
• Experience as MDS Coordinator in a skilled nursing or post‑acute setting preferred.
• Strong knowledge of the MDS 3.0 process and long‑term care regulations.
• Excellent organizational and time‑management skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Coordinate and oversee the facility's MDS assessment and care planning process.
• Ensure MDS assessments are completed accurately and within required timeframes.
• Collaborate with nursing, rehabilitation, dietary, social services, and other interdisciplinary team members.
• Review resident medical records and clinical documentation for accuracy and completeness.
• Participate in care plan meetings and maintain individualized care plans.
• Monitor changes in resident condition and ensure proper documentation and assessments.
