Job summary

VNA Health Group is seeking a Home Visitor RN to deliver nurse home visits through the Nurse-Family Partnership program in Somerset County, New Jersey. The role targets first-time mothers, supporting healthier pregnancies and child development, with a focus on building trust within families and the community.

You will conduct in-home visits, assess health needs, provide counseling, and coordinate referrals, while collaborating with CFHI and other team members.

Qualifications

• 2–3 years of nursing experience; home visiting or maternal/child health preferred.
• Proficiency with computers and clear verbal and written communication.
• Bilingual English/Spanish required.

Responsibilities

• Conduct in-home visits delivering nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients.
• Assess and address the health and developmental needs of women and their families.
• Offer counseling and instruction to help families achieve targeted health and life goals.
• Build and maintain strong community relationships to facilitate client referrals and resource connections.
• Attend weekly team meetings, case conferences, and clinical supervision sessions.
• Mentor and precept new hires and support student nurses during home visits as needed.
• Ensure compliance with agency infection control and safety policies.
• Maintain professionalism in adherence to organizational policies on absenteeism and appearance.
