Qualifications

• Graduate of accredited Registered Nurse Program.
• Licensure as RN in Virginia or compact state.
• Must have valid driver's license and vehicle for home visits.

Responsibilities

• Manage nursing care and provide individualized care for patients of all ages.
• Assess, plan, implement, and evaluate patient care to achieve optimum outcomes.
• Complete age-specific admission and ongoing assessments to support the plan of care.
• Collaborate with the health care team and family to develop and revise the plan of care.
