Qualifications

• Graduate of accredited Registered Nurse Program.
• Currently licensed as Registered Nurse in Virginia or approved compact state.
• Must have valid driver’s license and auto insurance for home visits.

Responsibilities

• Manage nursing care and provide individualized care for patients of all ages.
• Assess, plan, implement, and evaluate patient care to achieve optimum outcomes.
• Collaborate with health care team and family to develop and revise the interdisciplinary plan of care.
• Administer and document treatments using electronic medical records.
• Coordinate care coordination, scheduling, and documentation for CMS-required forms.
