Job summary

Carilion Clinic is seeking an RN Case Manager for Home Health in Radford, VA. You will manage nursing care for patients of all ages, develop and implement care plans, and collaborate with the health care team to promote safe transitions of care.

Responsibilities include performing OASIS-like assessments, documenting in the EMR, and coordinating with scheduling, billing, and other departments to ensure quality patient outcomes. A valid VA RN license and driving capability are required.

Qualifications

• Graduate of accredited Registered Nurse Program.
• Strong critical thinking and decision-making skills.
• Ability to plan, implement and evaluate patient care plans.

Responsibilities

• Administers and documents treatments and medications using EMR.
• Collaborates with care team to develop and revise interdisciplinary care plans.
• Coordinates and performs home visits with appropriate visit frequencies.
• Provides patient and caregiver education and advocacy.
• Supports transitions of care and reduces rehospitalizations.
