Job summary

Carilion Clinic is seeking a Part-time RN Case Manager for Home Health in the Tazewell, VA region. The successful candidate will manage nursing care, assess and plan care, and collaborate with healthcare teams to ensure optimal patient outcomes.

Responsibilities include documenting treatments in EMR, coordinating with families, and guiding LPNs and aides. A Virginia RN license, valid driver’s license, and experience with growth/development knowledge are required.

Qualifications

• Graduate of accredited Registered Nurse program.
• Currently licensed as RN in Virginia or a compact state.
• Valid driver's license and auto insurance for home visits.

Responsibilities

• Provide nursing care and individualized care planning for home health patients.
• Administer and document treatments and medications using EMR systems.
• Coordinate care with physicians, patients, and families to develop the plan of care.
• Supervise LPNs and Home Health Aides as required by guidelines.
• Participate in performance improvement activities and care coordination.
