Job summary

HCA Florida Capital Hospital in Tallahassee is looking for an RN Case Manager Part Time to coordinate patient care and support safe transitions across the continuum of care.

You will collaborate with physicians, nurses, and department leaders, developing individualized plans and guiding families through education on Medicare and insurance benefits while maintaining high-quality, patient-centered service in line with HCA Healthcare values.

Qualifications

• Associate Degree required.
• APRN or RN credential required.

Responsibilities

• Guide patients and families through program orientation, explain rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to participate in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths, needs and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, document findings and communicate needs across the care team.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare’s values through every patient and family interaction.
